In consideration of being permitted to participate in roller hockey activities, I hereby release and hold harmless, on my behalf and/or behalf of my child, his/her other parent, representatives, heirs, and assigns, XTREME WHEELS INLINE HOCKEY LEAGUE, VOODOO HOCKEY CLUB, XTREME WHEELS SKATING CENTER, Hunterz, Inc., their agents and employees, from all claims arising out of the sport of roller hockey. I accept full responsibility for the cost of treatment for any injury suffered while taking part in the program and agree to idemnify Hunterz, Inc. D.B.A., X-TREME WHEELS for any damages it may pay for in regards to me or on behalf of said child. I understand and acknowledge that as a participant engaging in activities that could invovle the risk of serious injury including disability or death, and severe social and economic losses which may result not only from their actions, inaction's or negligence, but the actions, inaction's, or negligence of others, the rules of play, or the conditions of the premises and/or any equipment used.
